Affidavit of Eligibility, Liability and Publicity Release

The signer of this affidavit verifies the originality, authorship and ownership as represented in the application, and the compliance with the LaunchPad Official Rules and Regulations. Maine Community Bank, Grove, the Judges, and their respective directors and employees are hereby released from all liability, claims, damages and injuries resulting from the Entrant’s participation in LaunchPad and the acceptance and use of a prize including without limitation, any personal injury, death, loss or damages of any kind or claims based on publicity rights, defamation, or invasion of privacy. Entrant agrees to return any prize received if the representations made herein prove to be untrue and to further indemnify and hold harmless Maine Community Bank, against any and all claims, damages and expenses, including attorney’s fees incurred as a result of any such misrepresentation. Entrant releases Maine Community Bank from any liability whatsoever for any claims, costs, injuries, losses or damages of any kind arising out of or in connection with participation in this program, or the downloading or accessing of Contest or entry materials, or the acceptance, possession or use/misuse of any prize awarded herein.

Entrant grants Maine Community Bank permission to use their name, likeness and personal information in any media relating to their participation for publicity, promotion, advertising and other purposes without further notice or additional compensation to promote LaunchPad, the idea and Maine Community Bank. Entrant agrees to make themselves available for interviews with the media in association with promoting the LaunchPad program.

By reading and agreeing to the terms of this affidavit, Entrant fully understands and unconditionally agrees to the LaunchPad Official Rules and Regulations.
